Catheter angiography required a Sim2 catheter and proved extremely difficult due to marked aortic tortuosity, a type III aortic arch and further tortuosity of the internal carotid artery (ICA) (Figure 1a). Despite this angiography via the common carotid artery did manage to successfully show a large aneurysm arising from the cavernous segment of the ICA (Figure 1b).
